No, I would not say the doctors only serve the super rich. There is not private system. Even if the super rich wanted to hoard all the doctors they could not.

Is private supplemental insurance illegal in Canada?
at least in UK, one can purchase private supplemental insurance
If I were stuck being a doctor in Canada, I would only accept cash patients even if it means being a bootlegger

This is twice as much better than India. Canada has two doctors for every thousand people. Many are Indians, so immigration is necessary. India produces 70000 doctors every year but half of them go to either Europe, Australia or NA.
The problem imo is that Canada provides so much liberty of practice to doctors. I've read somewhere that half of the doctors are just family doctors, means serving super rich people. Is that corect?
