LOL, that's the federal financial aid form. Just Google your university and use years in search terms or look for archived pages. All universities put everything online in the early 2000s, so you can find it. I'm guessing your school had multiple offices unless it was a private or smaller school.

My school had so many different offices. It was unbelievable! I believe we had a cultural diversity office.
