when you say medical care, I distinctly think it's the same as a doctor. Healthcare, hospital and medic really sound the same.

a docktor is there to contract with your person (contract an illness), to keep you indebted and sick. operates in commerce under admiralty law
a medic is what you mean
then you find medical care and pay for it, a docktor is not there to help you
