It's almost always social engineering. I feel like these companies should have social engineering courses.
Work in the IT-Sector in Germany. All companies I work for should have such. Some of them use Phishing campaigns for internal user audits. But even if the users failed before and get a webinar for phishing awareness, they fail again and again and again.

It might just be poor incentives then. Maybe if they got a bonus for passing those audits, they would pay more attention.
