I guess that's as hard of a problem like filtering spam since you essentially want to filter out even more things, not just "spam" (whatever you consider spam)

Yes, but enforcing topic uniformity would be nontrivial, ie there's nothing built in to the protocol that enforces that a note conforms to a topic.

Could you make a site or app that is subject specific? Like, I want to use nostr, but this site/app is only for discussing rock climbing or cooking. Then anyone on nostr can participant, but the site/app would only show those topics?
