My biggest problem has been consistency of quality - especially with shirts. It's pretty disappointing when you get something with an awesome design made on horrible material that barely fits and immediately shrinks.
That said, for a combo of both - I really like Phneep's designs. He takes old-time war propaganda posters and repurposes them for Bitcoin swag. Highly recommend.

This. I would love to find high quality, business casual type clothing with subdued design. Don't get me wrong, I love shirts that say "Fuck Fiat" but I can't wear those everywhere.
